False Positive Results in SARS-CoV-2 Serological Tests for Samples From Patients With Chronic Inflammatory Diseases

Patients with chronic inflammatory diseases are often treated immunosuppressants and therefore of particular concern during the SARS-CoV-2 pandemic. Serological tests will improve our understanding infection immunity in this population, unless they give false positive results. The aim study was to evaluate specificity SARS-Cov-2 serological assays using samples from patients collected prior April 2019, thus defined as negative. Samples multiple sclerosis (MS, n=10), rheumatoid arthritis (RA, n=47) or without factor (RF) and/or anti-cyclic citrullinated peptide antibodies (anti-CCP2) systemic lupus erythematosus (SLE, n=10) RF, were analyzed for 17 commercially available lateral flow (LFA), two ELISA kits one in-house developed IgG multiplex bead-based assay. Six LFA validated assay correctly produced negative results all samples. However, majority (n=13), gave signal RA SLE. This most notable RF patients. No detected any MS. Poor commercial could possibly be, at least partly, due interfering diseases. For these patients, risk positivity should be considered when interpreting assays.
